Neuromodulator treatment, which includes Botox and Dysport is available from our Nurse Practitioner through Hormones and Health.
These visits are IN PERSON in Oak Bluff. The exact location will be provided to you once your appointment is confirmed. Consent and health history form will be provided to you at the appointment
Fees: no refunds if cancelled 48hrs or less before appointment
If cancellation >48hrs your consultation fee will be used toward your rescheduled appointment if it is within 3 months. If it is >3months, or not rescheduled, then no refund or credit provided.
Fee for consult: with NO treatment: $100 for 30min in person
Fee for consult WITH treatment: $100 required to confirm appointment. This will be credited toward your treatment cost. This is a 45min appointment.
Consultation will be done. Client will identify which treatment areas she/he want to address.
The Nurse Practitioner will calculate how many units of Dysport/Botox are recommended. The client will be advised as to the anticipated treatment cost PRIOR to any injections.
Note: Payment for Dysport and Botox is calculated PER UNIT. The average number of units required depends on the treatment areas and size you want addressed.

The average # of units for glabellar and forehead may be 40-50 units; crow’s feet between 20-30 units; lower face (DAO) 10 units, chin 4-6 units.
IF the client agrees, then the treatment will be completed. Consent forms, photos, and records will be kept within the client record.
The client will receive post treatment guidelines.
Cost per unit: available upon request when you book your appointment.
Payment: consultation fee due at time of booking the appointment. Payment for treatment (cost per unit) due at the time of the appointment. This can be via EMR system (Hormones and Health website) or etransfer.
Please note there may be additional units required at the follow up consultation. If any additional units are recommended, the client will need to pay as per set fee per unit prior to receiving any injections.
